Herpessimplexencephalitistypicallymanifestsinolderadults(about50%ofcases)withheadache,fever,alteredsensorium,andevenseizure.Axial(topleft)andcoronal(bottomleft)T2-weightedFLAIRMRimagesshowthesitesofinfectionasincreasedsignalintensityintherightinsularcortexandrighttemporallobe,withcharacteristicsparingofthelentiformnuclei.Coronalanatomicsection(bottomcenter)showsparenchymaledemaandnecrosisincorrespondingareasofthebrain.Photomicrograph(bottomright)withhematoxylin-eosinstainshowsaglialnucleuscontaininganintranuclearCowdrytypeAinclusionbody,classicforherpesvirusinfection.

单纯疱疹病毒性脑炎

常见于老年人（约50％的病例）伴有头痛，发烧，感觉异常甚至癫痫发作。

轴位和冠状T2-FLAIRMR图像（左上及左下图）示感染部位（右侧岛叶皮质和右侧颞叶）病变信号强度增加，豆状核未受累为特征性表现。

冠状解剖（下中图）示脑部相应区域的实质水肿和坏死。

苏木精-伊红染色的镜下病理（右下图）示含有核内CowdryA型包涵体的神经胶质细胞核，是疱疹病毒感染的典型表现。
